During your nutrition consultation appointment you will receive the following reports and resources:
Wellness Suite staff are not registered dietitians, and a nutrition consultation should not be considered an adequate substitution for those seeking in-depth nutritional information, especially if you suffer from a health condition or nutritional deficiency. A registered dietitian (RD) is licensed by the State to provide nutritional counseling for individuals with specific medical concerns such as diabetes or an eating disorder. The Wellness Suite in conjunction with the UM health center can provide you with a referral to a local dietitian for this service.
In order to make an appointment you must first complete and submit a Nutrition Education Packet. The packet contains brief nutrition and fitness questionnaires, as well as a 3-day food recall log of all the food and drinks you consume on those days. It is recommended that you include two typical weekdays and a weekend day in your log.
Once you have completed the packet, please visit the Herbert Wellness Center Sales Office Monday-Friday, 7:30 a.m. - 8:00 p.m. or call 305-284-5433 to schedule your appointment.
Payment should be made when you submit you packet to the Wellness Suite. A Wellness Suite staff member will contact you no sooner than one week from the date of packet submission to arrange your appointment time. During the week prior to your appointment, staff will analyze your questionnaires and food log in preparation for your appointment.
